By completing this application form as legal guardian of the vacation bible school Participant listed above, herein known as the first party. I give permission for the said participant to attend the Riverside Baptist Church Vacation Bible School, June 10th through 14th 2024. I also release Riverside Baptist Church, second party, from any claims or causes of action, which may accrue while a the event mentioned above. Therefore, I hereby remise, release, acquit, satisfy, and forever discharge the said second party, of and from all and all manner of action and actions, cause and causes of action, suits, debts, dues, sums of money, account reckonings, bonds, bills, specialities, covenants, contracts, controversies, agreements, promises, variances, trespasses, damages, judgements, execution, claims, and demands whatsoever, in law o in equity, by direct claim or by indemnification, which said first party ever had, now has, or which any personal representative, successor, heir or assign of said first party, hereafter can seal or may have, against said second party, for upon or by reason of any matter, cause of thing whatsoever, from beginning of the world to the day of the presents.
I also acknowledge by registering my child to attend Vacation Bible School; I grant Riverside Baptist Church permission to use photos/videos of my child on their website, newsletter, bulletin boards, emails, media and special projects. I understand my child’s name will not be used without my written permission, and the pictures/videos will be used exclusively for Riverside Baptist Church purposes. The Vacation Bible School leader may also take pictures or videos of activities that may include my child along with other children in the class, which may be shared.
Children must be signed into and out of their classes by an Adult ( 18 and older) and cannot be signed in before 8:50 AM and must be picked by Noon.
Submission of this form holds space for you child. If your child cannot attend please contact Riverside Baptist Church so the space can be opened for another child.
Riverside’s Safety Policy
In order to provide a quality, safe experience for our children, those who miss the May 26th 2024 enrollment deadline will be accepted only if space is available. Availability will be determined the first morning of Vacation Bible School, June 10, 2024.
